Healthy Skin Membership Terms & Conditions

1. Membership Duration: Minimum commitment of 6 months required. Use your benefits on day one. Membership automatically renews monthly after initial term unless canceled with 30 days' written notice.

2. Billing: $125 is automatically charged monthly to the card on file. The $420 initial payment covers your first visit and begins your treatment cycle.

3. Treatment Frequency:

   • Radiofrequency Microneedling with PRF Plasma: 1 session every 3 months.

   • Neurotoxin: 1 treatment every 3 months.

   • Sculptra: Optional up to 4 treatments/year. Member receives $100 discount per syringe.

   • Dermal Filler (RHA, Versa, Restylane, Radiasse): Optional up to 4 treatments/year. Member receives $100 discount per syringe.

   • PRF Plasma Undereye Circles: Optional 1 Free treatments/year.

4. No Rollovers: Unused monthly services do not roll over. Appointments must be scheduled within your active membership cycle.

5. Cancellations & Rescheduling: 24-hour notice required to cancel or reschedule. Late cancellations or no-shows may result in a forfeited treatment for that cycle.

6. Non-Transferable: Membership is non-transferable and valid only for the enrolled individual.

7. Eligibility: Must be medically cleared to receive injectable treatments. A consultation is required prior to the first treatment.

8. Refund Policy: Membership fees are non-refundable once billed. Early termination before the 6-month minimum will incur a cancellation fee of $150.
